To apply for a Credit Card, use the New Consolidated P-Card Application, Agreement & Update Form:

We’re excited to announce the release of the updated P-Card Form, designed to simplify your experience. You can now use this single form to:

Apply for a new P-Card or Toshiba Ghost Card
Update changes to the approving official
Request cancellation of existing card(s)

This streamlined format replaces multiple previous forms, making it easier and faster to manage your P-Card needs. Please begin using the new form immediately for all related requests.
